Follow up report
The follow-up report is an essential tool for maintaining the link between JAME and the matched children, through the volunteer reader. It is also very useful when we communicate with principals and teachers to give them an overview of the progress of the child-volunteer match.
All volunteers of the organization commit to complete this report every 8 meetings, either electronically or by mail. A "paper" form is also available at our offices.
To complete the online monitoring report, please follow the steps below:
- Download the "Monitoring Report" form below;
- Right-click and then "save as";
- After saving it to your computer, fill in the appropriate boxes and save regularly;
- Once you have completed the form, send it to us by email as an attachment to your message.
